From the Field: Reclaiming Dignity: The Human Heart of ASPIRE
India— a vivid cultural landscape unlike anything in the West—from graffiti-covered buildings to lively street scenes that make each day feel like a celebration.
But beneath this beauty lies a deeper story of hidden health challenges, inequitable systems and resilience.
The Problem Nobody Wanted to See
In Northern India, leprosy and lymphatic filariasis (LF) are widely denied. Many people, especially those in rural villages, suffered in silence from diseases they didn’t even know existed, hidden away by stigma, misinformation, and a lack of access to care.
In the district of Jamui, home to 1.7 million people, this silent crisis required critical attention. That’s where our ASPIRE project stepped in.
A Model Built on Compassion and Community
Launched in 2021, we partnered with the Government of Bihar, LEPRA UK and LEPRA Society India to bring forth an innovative model of integrated care to reach those overlooked by traditional health systems in Jamui.
Together, we set out to change how skin-related NTDs are identified, treated, and understood—beginning at the community level.
ASPIRE’s approach is anchored in three core pillars:
- Active Case Finding: Field officers go door-to-door using visual aids to recognize early symptoms, aided by local female health workers to build trust.
- Health System Strengthening: Government clinics offer biweekly treatment. Doctors are trained to diagnose and care for skin-NTDs.
- Community Empowerment: Through 52 self-help groups, people affected learn how to care for themselves, support one another, and educate their communities.
Healing Beyond the Clinic
During our visit, we met people who had been rejected by their families and communities, ashamed of their disease. With ASPIRE, they found more than treatment—they found hope, respect, and a sense of belonging.

By the Numbers: Our Impact
In just two years, major progress was made in Jamui:
- 86% of households reached
- 1.2 million people screened for skin-related NTDs
- 1,624 leprosy cases diagnosed, 2,836 cases of scabies treated, and 3,612 people referred for LF treatment
- Estimated 2 million people reached through community awareness campaigns
- Over 2,000 healthcare workers trained
- 52 active self-help groups
Looking Ahead: A Blueprint for Global Change
With India accounting for over 60% of the world’s leprosy cases, projects like ASPIRE offer a path forward.
At a recent WHO Global Meeting in Geneva, Effect Hope’s Guillermo Robert, presented ASPIRE as a proven model to help meet the WHO’s 2030 elimination goals. Its success in Jamui is more than a local win—it’s a signal to the world that community-based, person-centered approaches can accelerate the global fight against NTDs.
